Avtech 717D problem

If I connect a camera to any of the CH 1-12 inputs on the rear of my DVR the picture is fine.

 

If I connect the same camera to any of the CH 13-16 I cannot see the picture at all just green and black horizontal lines.

 

When I try to playback the recorded video from any of the CH 1-12 all I can see are huge coloured blocks that change colour (I assume that this is the video being played back). There is also a mixture of solid green/white boxes and oblongs that hide the playback video in certain parts.

 

I have tried a soft and hard reset to the DVR with no luck. I have also tried to reinstall the firmware using a USB flash drive formatted to FAT 32 but the DVR will not recognise the flash drive.

 

I hope someone can help me solve this problem and I am sorry if I have put this thread in the wrong place.
