Please help to identify this board camera

I got this camera used from some guy on the net. It is color board camera, powered by 12V and supposed to be 500+ Horizontal TVL

Anyone can help me identifying this camera? (I really need manual for it, especially for these dip switches)

Watec make a cs mount board camera, it could be one of theirs

could be 1 of hundreds of different OEM board cams out of Asia ... I looked at the major US OEM brands but doesnt match.
