You need a Media Converter. From RJ45 convert to fiber optics. And 20km you need to use a single mode type.

I've seen one setup where a cable modem was installed inside a weatherproof box and given a static IP. Power cables where appropriate, a network camera, network cables, & etc... you get the idea.

I have come across 5km cameras but I think only thermal imaging right now in the market is possible to achieve that kinda range.
