I lost Password of my H.264 nETWORK DVR

Can someone help me. I have H.264 Network DVR and unfortunately something is wrong my box is locked I tried my password several times but it want open.

 

can someone tells me the correct username and password to enter.

 

I have tried everything but no opens any factory reset???
